Great START Application Information
To apply for a Great START Wage Supplement, please submit:
- Two application forms—the Participant Application and Great START Supplement Application (see link below).
- Both applications must be printed on white paper, completed, signed in blue or black ink.
- Original applications (no photocopies accepted) and all required documentation must be submitted for wage supplement consideration.
To renew a Great START Wage Supplement, please submit:
- Two application forms—the Information Update Form and Great START Supplement Application (see link below).
- Both applications must be printed on white paper, completed, signed in blue or black ink.
