How do I apply?

 

To Apply Online

The online application is a TWO PART process. To ensure that your complete application is ready for processing, you need to submit both parts.

Part One:

  1. Become a Gateways Registry Member and log into your Registry Dashboard.
  2. If your contact and employment information has changed, choose Update.
  3. After you have updated your information, go back to the My Registry and choose Apply.
  4. Select the Great START tab and choose Apply Now.
  5. Complete the steps and choose Submit Application.

Part Two:

  1. Once you have submitted your online application, you will see the Online Application Packet. Print this packet, sign it, and have your director sign it, if applicable.
  2. Read the list of required documents. Gather and email or mail all required documents and the Online Application Packet.

After your complete application is received and reviewed, you will be notified of your eligibility by email.

To Apply by Paper

  1. Be a Gateways Registry member. If you are not already a member, register here.
  2. Complete the  pdf Great START Application and Information Update Form. (1.02 MB)
  3. Read the list of required documents. Gather and email or mail all required documents and the Online Application Packet.

Required Documents

Please use the checklist provided with the application. Any missing documentation will delay the application process and could lead to being ineligible for the Great START Program. Upon review of an application, additional documentation may be required.

All Applicants

  • Completed and signed Registry Membership Form or, Information Update Form
  • Completed and signed Great START Supplement Application
  • Signed W-9 (IRS Form) with applicant's information.

Center Staff and Family/Group Home Child Care Assistant Applicants

  • Four weeks of most recent pay stubs
  • Most recent W-2 (IRS Form)

Family/Group Child Care Home Director/Owner Applicants

  • Most recent Schedule C (IRS Form)
  • Verification of children currently being served with completed IDHS child care assistance billing certificates/program verification OR Proof of Care Form
  • Most recent Form 8829 (IRS Form) with line 4 completed OR updated parent handbook, indicating hours and days of operation and holiday/vacation schedule
